Pain Management in ESKD Patients

Safe Alternative Pain Management

  • The American College of Physicians recommends acetaminophen as the safest first-line analgesic, with a maximum dose of 3000 mg/day, for patients with end-stage kidney disease (ESKD) 1
  • The National Kidney Foundation suggests using topical agents, such as lidocaine 5% patch or diclofenac gel, for localized pain without systemic absorption in ESKD patients 1, 2
  • For neuropathic pain, the European Renal Association recommends gabapentin or pregabalin, with careful titration and significant dose adjustment in ESKD patients 1, 2
  • The Mayo Clinic recommends avoiding morphine, codeine, meperidine, tramadol, and tapentadol in ESKD patients due to accumulation of active metabolites, and instead using fentanyl, buprenorphine, or methadone 3

Non-Pharmacological Options

  • The American Heart Association suggests that exercise programs and local heat application can provide significant relief without affecting renal function in ESKD patients 1, 2
