Estrogel Dosing Guidelines

Standard Dosing Framework

  • The American College of Obstetricians and Gynecologists recommends starting with 0.5-0.75 mg daily (approximately 1 pump of Estrogel 0.06%) applied to clean, dry skin on the lower abdomen, upper outer arm, or thighs 1
  • The maximum approved dose is 1.5 mg daily (approximately 2 pumps), which delivers an estimated dose of estradiol through transdermal absorption, although the exact amount is not specified 1

Critical Endometrial Protection Requirement

  • The North American Menopause Society recommends adding micronized progesterone 200 mg orally (or vaginally) for 12-14 days every 28 days to prevent endometrial hyperplasia and cancer in women with an intact uterus 1, 2
  • Alternative regimens include 10 mg medroxyprogesterone acetate for 12-14 days monthly 2
  • Failure to add progestin is a critical prescribing error that can lead to endometrial cancer 1

Application Instructions and Common Pitfalls

  • The manufacturer recommends applying Estrogel to clean, dry skin on the lower abdomen, upper outer arm, or thighs, rotating application sites with each use to minimize skin irritation, and allowing the gel to dry completely before dressing 1
  • The guideline society recommends against starting with high doses (>1 mg daily) as initial therapy, as evidence shows no additional benefit and increased harm 1
  • The society also recommends against prescribing estrogen solely for chronic disease prevention in asymptomatic postmenopausal women, as risks outweigh benefits 1, 3
