Lamotrigine Reinitiation Protocol
Criterios Críticos para Carga Rápida
- The American Academy of Emergency Medicine recommends that lamotrigine can be reinitiated with a single loading dose of 6.5 mg/kg orally only if the patient has been taking lamotrigine for more than 6 months, has no history of rash or previous intolerance, and has been off the medication for less than 5 days 1, 2, 3
- Patients must meet all the following criteria: duration of previous treatment >6 months, interruption <5 days, no previous rash, and no documented intolerance 1, 2, 3
Contraindicaciones Absolutas para Reinicio Rápido
- The American Academy of Emergency Medicine states that a history of previous rash with lamotrigine is an absolute contraindication for rapid reinitiation due to the high risk of severe cutaneous reactions, including Stevens-Johnson syndrome 1, 2, 3
Interacciones Medicamentosas
- The guideline society recommends that when using lamotrigine with enzyme inducers (phenytoin, carbamazepine, barbiturics, topiramate, oxcarbazepine), higher doses may be required 5
- Concomitant use of combined hormonal contraceptives may reduce the effectiveness of lamotrigine, and higher doses or alternative methods should be considered 5
Consideraciones de Formulación
- The guideline society notes that regular and dispersible tablets have identical bioequivalence, and no dose adjustment is required when switching between formulations 5, 6
- Dispersible tablets can be useful for patients with swallowing difficulties, as they dissolve quickly in the mouth without water 6
