Carbamazepine Interactions

Introduction to Carbamazepine Interactions

  • Carbamazepine is a strong inducer of Cytochrome P450 enzymes (especially CYP3A4 and CYP2C9) and can significantly reduce the efficacy of many concurrently administered medications, while certain medications can increase or decrease carbamazepine levels themselves 1

Effect of Other Medications on Carbamazepine

  • The European Society of Cardiology recommends caution when using carbamazepine with medications that may increase its levels, such as CYP3A4 inhibitors, which can lead to toxic concentrations 1

Effect of Carbamazepine on Other Medications

  • The American Heart Association notes that carbamazepine can reduce the efficacy of certain cardiovascular medications, such as digoxin, and recommends therapeutic monitoring 2
  • The European Association for the Study of the Liver reports that carbamazepine can interact with antiretroviral medications 3
  • The European Society of Cardiology recommends avoiding the combination of carbamazepine with certain medications, such as amiodarone, due to the risk of life-threatening arrhythmias 3
  • The American College of Cardiology notes that carbamazepine can significantly reduce the levels of sildenafil through CYP3A4 induction 4

Special Precautions

  • The European Respiratory Society recommends that workers exposed to chemical substances should be cautious when taking carbamazepine, as it may increase the risk of liver toxicity by promoting the formation of reactive metabolites from other chemicals 1
  • The European Society of Cardiology recommends special caution in patients with pulmonary hypertension who are receiving PAH-targeted therapies 4, 5
  • The American Heart Association recommends caution when using carbamazepine with antihypertensive medications due to the risk of excessive systemic hypotension 4

Driving Ability

  • The National Highway Traffic Safety Administration notes that acute administration of carbamazepine can impair driving ability 6, 7, 8
  • The American Academy of Sleep Medicine reports that chronic use of carbamazepine does not appear to significantly impair driving ability 6, 7, 9
  • The National Institute of Neurological Disorders and Stroke recommends that patients be informed about possible impairments in driving ability, especially at the beginning of therapy 7, 8
