Nitrofurantoin Dosing Considerations in Adults with UTIs

Standard Dosing and Efficacy

  • The American College of Physicians and the Infectious Diseases Society of America recommend nitrofurantoin monohydrate/macrocrystals (Macrobid) 100 mg twice daily for 5 days as the first-line regimen for uncomplicated urinary tract infections, with clinical cure rates ranging from 88-93% and bacterial cure rates ranging from 81-92% 2, 3, 4
  • The European Association of Urology also recommends nitrofurantoin as a first-line agent for uncomplicated cystitis 5

Renal Function Considerations

  • Nitrofurantoin is contraindicated in patients with creatinine clearance <60 mL/min due to inadequate urinary drug concentrations and increased risk of toxicity, including peripheral neuropathy and other toxicities 1
  • The European Association of Urology and other guideline societies recommend considering alternative agents, such as trimethoprim-sulfamethoxazole or fosfomycin trometamol, for patients with CrCl <60 mL/min 3, 5, 6

Alternative Agents and Monitoring

  • For patients with CrCl <60 mL/min, alternative agents such as trimethoprim-sulfamethoxazole 160/800 mg twice daily for 3 days or fosfomycin trometamol 3 g single dose may be considered, with fluoroquinolones reserved for more invasive infections due to concerns about resistance and collateral damage 3, 5, 6
  • The European Association of Urology recommends routine monitoring for signs of toxicity, including peripheral neuropathy, pulmonary reactions, and hepatotoxicity, in patients with borderline renal function 1, 5
