Itraconazole Pharmacology: Dosing and Duration for Fungal Infections

Dosing by Infection Type

  • The National Comprehensive Cancer Network recommends itraconazole dosing varies significantly by infection type and severity, with most systemic infections requiring 200 mg once or twice daily for 6-12 months, though therapeutic drug monitoring is essential to ensure adequate exposure with target trough levels of 1-2 mcg/mL for invasive infections and >0.5 mcg/mL for prophylaxis 1
  • For oropharyngeal candidiasis, the Journal of Microbiology, Immunology and Infection suggests 200 mg once daily for 7-14 days for uncomplicated disease 2, 3, 4
  • For esophageal candidiasis, the Journal of Microbiology, Immunology and Infection recommends 200 mg once daily for 14-21 days until clinical improvement 2, 3, 4, 6
  • For blastomycosis, Clinical Infectious Diseases suggests 200 mg three times daily for 3 days (loading dose), then 200 mg once or twice daily for 6-12 months 7
  • For moderately severe to severe blastomycosis, Clinical Infectious Diseases recommends initial amphotericin B for 1-2 weeks until improvement, then step-down to itraconazole 200 mg three times daily for 3 days, then 200 mg twice daily for total duration of 6-12 months 7

Critical Pharmacokinetic Considerations

  • The National Comprehensive Cancer Network recommends target trough concentrations of >0.5 mcg/mL for prophylaxis and 1-2 mcg/mL for active invasive infection 1
  • The National Comprehensive Cancer Network suggests measuring serum levels after at least 2 weeks of therapy to ensure adequate drug exposure 1
  • Clinical Infectious Diseases recommends measuring serum levels after at least 2 weeks of therapy to ensure adequate drug exposure 7

Special Populations

  • For immunocompromised patients, Clinical Infectious Diseases suggests initial therapy with amphotericin B for 1-2 weeks until improvement, then step-down to itraconazole 200 mg three times daily for 3 days, then twice daily 7
  • For CNS infections, Clinical Infectious Diseases recommends itraconazole 200 mg 2-3 times daily for at least 12 months and until resolution of CSF abnormalities 7
