Management of Hematological Disorders

Initial Diagnosis and Treatment

  • The National Comprehensive Cancer Network recommends initiating tyrosine kinase inhibitor therapy based on risk stratification 1, 2
  • Monitoring of blood counts should be done weekly during the first 4-6 weeks of therapy, then every 2 weeks until month 3, followed by every 3 months, as suggested by the Leukemia journal guidelines 3

Treatment of Symptomatic Disease

  • The National Comprehensive Cancer Network suggests considering purine analog therapy if symptomatic disease is present 4
  • Anemia management involves various strategies, including transfusion therapy, erythropoiesis-stimulating agents, and iron overload management, as recommended by the American Society of Clinical Oncology and the National Comprehensive Cancer Network 5, 6
  • Transfusion of red blood cells is recommended for symptomatic anemia or hemoglobin <7-8 g/dL, using a restrictive transfusion strategy to minimize complications, as suggested by the American Society of Clinical Oncology, the American Society of Anesthesiologists, and the National Comprehensive Cancer Network 5, 7, 8
  • Erythropoiesis-stimulating agents may be considered for reducing transfusion dependence, particularly in patients with low-risk myelodysplastic syndrome, as recommended by the American Society of Clinical Oncology and the National Comprehensive Cancer Network 5, 6
  • Iron chelation should be considered when ferritin >1000 mcg/L in patients with frequent transfusions, as recommended by the National Comprehensive Cancer Network 6

Management of Neutropenia

  • For severe neutropenia (ANC <500/μL) or neutropenic fever, the American College of Clinical Pharmacy recommends considering G-CSF (filgrastim) at 5 μg/kg/day subcutaneously, as well as immediate broad-spectrum antibiotics for neutropenic fever 9
  • Erythropoiesis-stimulating agents and granulocyte colony-stimulating factors should be considered for patients with persistent leukopenia, particularly when white blood cell counts fall below 1000 cells/μL or when patients develop neutropenic fever, as recommended by the American College of Hematology 10
  • Regular CBC monitoring every 2-4 weeks in patients with leukopenia is crucial, as recommended by the American College of Physicians 10
  • Avoid starting lenalidomide in patients with leukopenia due to risk of further myelosuppression, as advised by the American Society of Hematology 10

Diagnostic Evaluation

  • Initial evaluation of leukopenia should include:
  • The National Comprehensive Cancer Network, American College of Pathology, and International Society for Laboratory Hematology recommend the following diagnostic tests:

Risk Stratification

  • The International Prognostic Scoring System (IPSS) should be used for risk stratification in myelodysplastic syndrome (MDS), as recommended by the National Comprehensive Cancer Network, the European Society for Medical Oncology, and the International Society for Experimental Hematology 6, 14, 12

Prevention of Complications

  • Folic acid supplementation (1 mg daily) should be considered, as recommended by the American Society of Clinical Oncology 8
  • Recognize neutropenic fever as a medical emergency requiring immediate intervention with broad-spectrum antibiotics, as emphasized by the Centers for Disease Control and Prevention
  • Do not attribute leukopenia solely to COVID-19, without excluding other serious causes including underlying hematological malignancies, as warned by the World Health Organization
  • Reserve growth factors for severe neutropenia or neutropenic fever, as recommended by the American College of Hematology 10
  • Perform comprehensive evaluation for multiple cell lines, as suggested by the European Hematology Association 15
