Tramadol Administration Guidelines

Pharmacokinetics and Dosing

  • The dual mechanism of action of tramadol, as a weak mu-opioid receptor agonist plus norepinephrine and serotonin reuptake inhibition, functions independently of food intake, according to Praxis Medical Insights 1
  • Starting with the lowest effective dose, typically 50-100 mg every 4-6 hours, is recommended to minimize dose-dependent adverse effects, particularly nausea, as suggested by the Journal of the American Geriatrics Society 2
  • The maximum daily dose is 400 mg for immediate-release formulations and 300 mg for extended-release formulations, as per Praxis Medical Insights 1
  • Reduced dosing is necessary for older adults (≥75 years) and those with hepatic or renal dysfunction, according to Praxis Medical Insights 1
