Management of Alcohol Withdrawal Syndrome Based on CIWA Score

Patient Assessment and Disposition

  • Outpatient management is appropriate for patients with mild withdrawal symptoms (CIWA <8) who have no significant comorbidities or complications, according to the Journal of Hepatology 1
  • Patients with CIWA scores ≥15 should be admitted for inpatient treatment due to high risk of severe alcohol withdrawal syndrome, as recommended by the Journal of Hepatology 1
  • The American Association for the Study of Liver Diseases recommends inpatient admission for patients with CIWA scores ≥15, indicating severe AWS, and high risk for complications including withdrawal seizures and delirium tremens 1

Pharmacological Management

  • Benzodiazepines are the first-line treatment for AWS with CIWA scores ≥8, as recommended by the Journal of Hepatology and Clinical and Molecular Hepatology 1, 2
  • Lorazepam (6-12 mg/day) is recommended for patients with severe AWS, advanced age, recent head trauma, liver failure, respiratory failure, other serious medical comorbidities, or obesity, according to Clinical and Molecular Hepatology 2
  • Thiamine supplementation (100-300 mg/day) should be given to all patients with AWS and maintained for 2-3 months following resolution of withdrawal symptoms, as recommended by Clinical and Molecular Hepatology 2

Monitoring and Treatment Approach

  • Daily laboratory monitoring including electrolytes and liver function tests is recommended for patients with CIWA ≥15 or with comorbidities, according to Clinical and Molecular Hepatology 2
  • Patient education regarding warning signs that would necessitate return to medical care is essential, as recommended by Praxis Medical Insights 3
  • Arrangement for follow-up within 24-48 hours is necessary for patients with CIWA <8, according to Praxis Medical Insights 3

Special Considerations

  • Psychiatric consultation is recommended for evaluation, treatment, and long-term planning of alcohol abstinence, as recommended by Clinical and Molecular Hepatology 2
  • Patients with a history of withdrawal seizures, delirium tremens, or failed outpatient detoxification should be considered for inpatient treatment regardless of initial CIWA score, according to Clinical and Molecular Hepatology 2
