Aortic Root Dilation Management

Introduction to Aortic Root Dilation

  • For a patient with an aortic root diameter of 4.5cm, close monitoring with regular imaging is recommended, with surgical intervention indicated only if specific risk factors are present or if the patient is undergoing aortic valve surgery, as recommended by the American Heart Association 1

Risk Factors for Aortic Dissection

  • Patients with Marfan syndrome and an aortic root diameter of 4.5cm should undergo surgical repair, according to the European Society of Cardiology 3, 4
  • Women with aortic root diameter of 4.5cm have approximately 10% risk of aortic dissection during pregnancy, and cesarean delivery is advised rather than vaginal delivery, as suggested by the European Heart Journal 3
  • Family history of aortic dissection, particularly if family members experienced dissection at diameters <5.0cm, is a significant risk factor for surgical intervention at a diameter of 4.5cm, as indicated by the American Heart Association 1
  • Bicuspid aortic valve (BAV) patients undergoing aortic valve repair/replacement should consider concomitant replacement of the aortic root at a diameter of 4.5cm, as recommended by the American Heart Association 1

Surveillance and Monitoring

  • Regular surveillance imaging is indicated for patients with an aortic root diameter of 4.5cm without additional risk factors, with recommended surveillance including echocardiography every 6-12 months, as suggested by the American Heart Association 1, 2
  • If growth rate ≥0.3 cm/year for 2 consecutive years or ≥0.5 cm in 1 year, surgical intervention is indicated regardless of absolute size, according to the American Heart Association 1
  • Transthoracic echocardiography (TTE) is recommended for initial and routine follow-up, with cardiac MRI or CT indicated if TTE visualization is inadequate, as suggested by the American Heart Association 1, 2
  • For accurate measurement of growth rate, use cardiac-gated CT or MRI with centerline measurement techniques, as recommended by the American Heart Association 1

Surgical Intervention

  • Concomitant replacement of the aortic root is reasonable if the patient requires aortic valve surgery and has an aortic root diameter of 4.5cm, as supported by multiple guidelines, including the American Heart Association 1, 2, 4
  • Surgical intervention is indicated for patients with rapid growth rate (≥0.3 cm/year) or cross-sectional aortic area/height ratio ≥10 cm²/m, as suggested by the American Heart Association 1

Pregnancy and Aortic Root Dilation

  • Counseling regarding the risk of aortic dissection during pregnancy is essential for women with aortic root diameter of 4.5cm, and cesarean delivery is advised rather than vaginal delivery, as recommended by the European Society of Cardiology 3
  • Consider more frequent monitoring during pregnancy for patients with aortic root dilation, as suggested by the European Heart Journal 3

Imaging Frequency

  • Annual imaging (echocardiography preferred) is recommended for stable aortic dimensions, as suggested by the American Heart Association 1
  • Imaging every 6 months is recommended for borderline or concerning features, according to the American Heart Association 1
