Management of Chest Pain in Women
Initial Evaluation
- The American College of Cardiology recommends immediately obtaining an ECG and performing a focused cardiovascular history in a 40-year-old woman presenting with chest pain, emphasizing accompanying symptoms that are more common in women with acute coronary syndrome 1, 2, 3
- The American Heart Association suggests obtaining a 12-lead ECG within 10 minutes if the patient presents to any healthcare setting with acute chest pain 2, 3
- If evaluated in an office setting without ECG capability, the American Heart Association recommends referring immediately to the emergency department 2
- If acute coronary syndrome (ACS) is suspected based on clinical presentation, the American Heart Association advises activating emergency medical services (EMS) for transport rather than having the patient self-transport 2, 3
Risk Stratification
- The American College of Cardiology states that women aged 40-49 with anginal symptoms have a pretest probability of <10% for obstructive coronary artery disease (CAD) 7
- However, the American Heart Association notes that this does not exclude ACS, particularly in the acute setting with concerning features 7
- Traditional risk scores overestimate disease probability in contemporary populations, but clinical judgment must prioritize safety, according to the American Heart Association 7
Diagnostic Considerations
- The European Society of Cardiology recommends measuring cardiac troponin (cTn) as soon as possible after ED presentation if ACS is suspected 2
- The American College of Cardiology advises against routinely ordering troponin in the primary care office setting for suspected ACS—instead, refer to the ED for definitive testing 2, 6
- High-sensitivity troponin (hs-cTn) protocols allow for rapid rule-out within 1-3 hours, according to the American College of Cardiology 2
Management
- The American Heart Association recommends administering aspirin 250-500 mg (chewable or water-soluble for rapid absorption) if ACS is suspected or cannot be excluded 5
- The European Society of Cardiology suggests considering sublingual nitroglycerin if no contraindications (hypotension, bradycardia, recent PDE-5 inhibitor use) 5
- However, the American College of Cardiology notes that relief with nitroglycerin is not diagnostic of cardiac ischemia and should not be used as a diagnostic criterion 1
- The American Heart Association advises transporting the patient by EMS to the ED for definitive evaluation if ACS is suspected or cannot be excluded 2, 3
Special Considerations for Women
- The American College of Cardiology states that women presenting with chest pain face systematic underdiagnosis because risk assessment tools underestimate cardiac risk in women, symptoms are misclassified as noncardiac, and "atypical" terminology is based on male presentation patterns 1, 2, 6
- The American Heart Association recommends maintaining a lower threshold for cardiac evaluation in women with any concerning features, particularly those with cardiovascular risk factors 1, 2, 6
