Management of Edema and Hypertension with Diuretics

Efficacy and Recommendations

  • The American College of Cardiology recommends loop diuretics, such as furosemide, as first-line therapy for edema management in heart failure, with a typical starting dose of 20-40 mg once or twice daily (maximum 600 mg/day) 1
  • For edema management in cirrhosis with ascites, the combination of spironolactone and furosemide is recommended, with a starting dose of 100 mg spironolactone and 40 mg furosemide to maintain normokalemia 2
  • In patients with resistant hypertension, the addition of spironolactone to a thiazide or loop diuretic regimen may be considered for enhanced efficacy 3, 4
  • For heart failure with reduced ejection fraction, guidelines recommend the addition of aldosterone antagonists, such as spironolactone, to loop diuretics, such as furosemide, for patients with NYHA class II-IV symptoms 1, 5

Monitoring and Adverse Effects

  • Monitoring of electrolytes, particularly potassium, is required for both furosemide and spironolactone 7, 5
  • Renal function should be monitored with either medication, as deterioration may require dose adjustment or discontinuation 5

Special Considerations

  • In patients with renal impairment, higher doses of furosemide may be needed, while spironolactone dosing may need to be reduced due to hyperkalemia risk 2
  • Spironolactone can cause gynecomastia in men, and amiloride can be substituted in such cases 2
