Developmental Screening and Surveillance at 18-Month Well-Child Visit

Developmental Assessment

  • The American Academy of Pediatrics recommends performing standardized developmental screening using a validated tool, such as Ages and Stages Questionnaire or Parents' Evaluation of Developmental Status, at the 18-month well-child visit 1, 2, 3
  • Formal standardized developmental screening is mandatory at 18 months, not just clinical observation or milestone checklists, as it helps identify developmental delays early 1, 2, 3
  • Use a validated parent-completed screening tool rather than relying on clinical judgment alone, which misses 45% of children eligible for early intervention 3
  • Critical motor milestones to observe at 18 months include sitting, standing, and walking independently, and grasping and manipulating small objects, with absence of these skills signifying delay requiring further evaluation 1, 2

Motor Assessment

  • At 18 months, gross motor achievements should include walking backward, running, and walking up steps with hand held, while fine motor achievements should include scribbling in imitation, dumping small objects from a bottle, and building a tower of 2 cubes 4
  • Motor symmetry should be present, with asymmetry warranting neurologic evaluation 1

Referral Criteria

  • Any child failing standardized developmental screening requires referral to early intervention programs 1, 3
  • Children with absent expected motor milestones, such as not walking independently or not grasping small objects, need further evaluation 1, 2
  • Loss of previously acquired skills mandates urgent neurologic evaluation for progressive disorders 4, 1

Neurologic Evaluation

  • Increased muscle tone warrants brain MRI, while decreased muscle tone warrants serum creatine kinase measurement 6
  • Assess muscle tone as part of neurologic examination, as abnormalities may indicate neuromuscular disorders 6, 1
