Management of Fatigue and Elevated LDL
Addressing Fatigue
- The American Heart Association recommends considering sleep quality assessment, depression screening, medication review, cardiovascular fitness assessment, and occult malignancy screening in patients with fatigue, particularly in postmenopausal women 2
- The American College of Cardiology suggests that iron deficiency can be present even with normal ferritin levels, and recommends checking iron saturation and TIBC to rule out iron deficiency 1
- A structured exercise program with moderate-intensity aerobic activity for 30-60 minutes on most days can improve functional capacity and reduce fatigue, as recommended by the American Heart Association 4, 2
Managing Elevated LDL
- The American College of Cardiology recommends calculating 10-year ASCVD risk using the pooled cohort equation to determine treatment intensity, and assessing for additional risk factors such as family history of premature CHD and metabolic syndrome components 4, 5
- The American Heart Association suggests that dietary modifications, including limiting saturated fat to <7% of total calories, restricting dietary cholesterol to <200 mg/day, and increasing soluble fiber to 10-25 g/day, can help lower LDL levels 4, 3
- The American College of Cardiology recommends initiating statin therapy if LDL remains ≥130 mg/dL with 10-year risk ≥10%, or if LDL remains ≥160 mg/dL with 10-year risk <10%, and selecting a statin such as atorvastatin 10-20 mg daily as first-line therapy 4, 7
- The American Heart Association recommends monitoring lipid panel at 6-12 weeks after initiation of statin therapy, and adjusting treatment as needed to achieve LDL goals 1
Follow-up Strategy
- The American College of Cardiology recommends reassessing patients in 6-12 weeks after initiating therapeutic lifestyle changes, and rechecking CBC with iron studies to clarify microcytic patterns 4, 1
- The American Heart Association suggests monitoring lipid panel every 3-6 months until LDL goal is achieved, then every 6-12 months 1
