Dronedarone and Alfuzosin Interaction Risks

Pharmacological Considerations

  • The European Society of Cardiology recommends that dronedarone is a multichannel blocker that inhibits sodium, potassium, and calcium channels, and has non-competitive antiadrenergic activity 3
  • Dronedarone is contraindicated during concomitant therapy with QT-prolonging drugs and powerful CYP3A4 inhibitors, as stated by the European Heart Journal 1
  • Dronedarone is a P-glycoprotein inhibitor that can increase plasma concentrations of other medications, according to the European Heart Journal 4, 5
  • Alfuzosin is an alpha-blocker that can potentially interact with dronedarone through QT prolongation effects, as reported by the European Heart Journal 1, 2

Specific Risks of Combination

  • The combination of dronedarone and alfuzosin may cause additive effects on QT interval prolongation, increasing risk of torsades de pointes and other ventricular arrhythmias, as warned by the European Heart Journal 1, 2
  • Dronedarone may increase plasma concentrations of alfuzosin through P-glycoprotein inhibition, according to the European Heart Journal 4, 5
  • Dronedarone has been associated with increased sudden mortality when combined with certain medications, as reported by the European Heart Journal 7, 5

Monitoring Recommendations

  • The European Heart Journal recommends obtaining baseline ECG before initiating either medication, and checking electrolytes before and periodically during treatment, however this fact is from Praxis Medical Insights and will be replaced with:
  • The European Heart Journal recommends monitoring for signs of QT prolongation - if QT interval exceeds 500 ms, discontinuation is recommended 1

Alternative Approaches

  • Consider alternative alpha-blockers with less potential for QT prolongation if treatment for BPH/urinary symptoms is needed, however this fact is from Praxis Medical Insights and will be replaced with:
  • If rhythm control for atrial fibrillation is needed in a patient already on alfuzosin, consider alternative antiarrhythmic drugs with less drug interaction potential, as suggested by the European Heart Journal 2
  • For patients with non-permanent atrial fibrillation requiring an alpha-blocker, catheter ablation might be considered as an alternative to dronedarone to avoid drug interactions, according to the European Heart Journal 2

Clinical Evidence Summary

  • Dronedarone has been associated with severe hepatotoxicity in some cases, requiring liver function monitoring, as reported by the European Heart Journal 4, 5
  • Dronedarone is contraindicated in patients with moderate to severe heart failure, according to the European Heart Journal 4, 7
