SSRI Selection Based on Sedation Effects

Introduction to SSRI Activation Profiles

  • The American Academy of Family Physicians recommends fluoxetine as first-line when an activating antidepressant effect is desired, particularly beneficial for patients with fatigue, hypersomnia, or psychomotor retardation 1
  • Fluoxetine is the most activating SSRI and therefore has the least sedating effects, making it the optimal choice when sedation needs to be minimized 1

Clinical Considerations for SSRI Selection

  • The American Academy of Family Physicians recommends fluoxetine for patients with fatigue, hypersomnia, or psychomotor retardation 1
  • Activating SSRIs like fluoxetine should be avoided in patients with significant anxiety, agitation, or insomnia, and in patients with seizure disorders due to potential lowering of seizure threshold 1

Special Population Considerations

  • The American Family Physician recommends avoiding fluoxetine and paroxetine in older adults due to higher rates of adverse effects, and instead prefers citalopram, escitalopram, and sertraline for older patients 2

Adverse Effects of SSRIs

  • Sexual dysfunction is a common adverse effect of SSRIs 2
