Clindamycin Use for Antibiotic Prophylaxis in Canada

Indications and Dosing

  • The Canadian medical community recommends clindamycin as an alternative antibiotic prophylaxis agent in abdominal procedures for patients with beta-lactam allergies, with a standard dose of 900 mg IV slow infusion for most procedures 1, 3, 5
  • For bariatric surgery, such as gastric bypass or sleeve gastrectomy, a higher dose of 2100 mg slow IV as a single dose is recommended, typically combined with gentamicin 2
  • Clindamycin is effective against streptococci, staphylococci, and anaerobic bacteria, which are common pathogens in abdominal procedures 1

Combination Therapy and Administration

  • Clindamycin is often combined with gentamicin to provide broader coverage against gram-negative organisms when used as an alternative to beta-lactams 1, 6
  • Additional 600 mg of clindamycin may be administered if the procedure duration exceeds 4 hours 3
  • The duration of clindamycin prophylaxis is generally limited to a single dose or the operative period only, with a maximum of 24 hours 5
