Somatic Manifestations of Depression in Elderly Patients

Diagnostic Considerations

  • Sleep disturbances are a common somatic manifestation of depression in elderly patients, according to the Journal of the American Geriatrics Society 1
  • The Geriatric Depression Scale (GDS) is specifically designed to assess depression in older adults, focusing on both positive and negative affective symptoms, as recommended by the Journal of Clinical Oncology 2, 3
  • The Beck Depression Inventory (BDI) includes assessment of behavioral, cognitive, and somatic components of depression, making it useful for identifying depression with significant somatic presentation, as noted by the Journal of Clinical Oncology 2, 3

Differential Diagnosis

  • Somatic symptoms of depression must be distinguished from side effects of medications, primary medical conditions, and primary sleep disorders, such as obstructive sleep apnea and restless legs syndrome, according to the Journal of the American Geriatrics Society 1
  • Insomnia in older adults is often comorbid with depression, with depressed individuals 2.5 times more likely to report insomnia, as reported by the Journal of the American Geriatrics Society 1

Screening Recommendations

  • The U.S. Preventive Services Task Force recommends recurrent screening for depression in patients with unexplained somatic symptoms, comorbid psychological conditions, substance abuse, or chronic pain, as published in the Annals of Internal Medicine 4, 5
