Treatment of Mild Croup in Children
Introduction to Croup Management
- The American Academy of Pediatrics and other medical societies recommend against the use of over-the-counter cough medications in children with croup, as they have little to no benefit for cough control and are associated with adverse events 1, 2
Non-Recommended Treatments
- Inhaled corticosteroids from hand-held inhalers with spacer devices are not effective for croup and should be avoided 3
- Nebulized epinephrine is reserved for significant respiratory distress in a hospital setting and has a short-lived effect with risk of rebound symptoms, and its use in the outpatient setting for mild croup is not recommended 3
Supportive Care for Croup
- For children with mild croup where steroids are absolutely contraindicated, supportive care only, including keeping the child calm, avoiding distress, ensuring adequate hydration, is recommended 3
- Monitoring for worsening symptoms, including stridor at rest, respiratory distress, or oxygen saturation <94%, is crucial in children with croup 3
- Reassessment within 24 hours if symptoms persist is necessary for children with croup 3
- Immediate medical attention is required if symptoms worsen, as this may indicate the need for hospital-based interventions 3
