Atracurium for Muscle Relaxation in General Anesthesia

Dosing and Administration

  • For airway obstruction related to supraglottic devices, a dose of 0.1-0.2 mg/kg of atracurium is recommended if the depth of anesthesia is adequate, as per the Anaesthesia guidelines 1, 2
  • The American Society of Anesthesiologists recommends monitoring the corrugator supercilii muscle using a peripheral nerve stimulator for optimal dosing, due to its sensitivity to muscle relaxants and kinetics comparable to laryngeal muscle 1, 3

Clinical Applications

  • Atracurium is recommended for abdominal laparotomy or laparoscopy surgery with a Grade 1+ recommendation, according to the Anaesthesia guidelines 4
  • Atracurium is probably recommended for ENT laser surgery with a Grade 2+ recommendation, as per the Anaesthesia guidelines 4
  • For patients with renal or hepatic failure, atracurium is probably recommended over other muscle relaxants due to its organ-independent elimination, with a Grade 2+ recommendation, as stated by the Anaesthesia guidelines 5, 6
  • No dose modification is required for atracurium in patients with renal or hepatic failure, as recommended by the Anaesthesia guidelines 5, 6, 7

Monitoring and Reversal

  • Intraoperative monitoring of neuromuscular blockade is recommended with a Grade 1+ recommendation, according to the Anaesthesia guidelines 1
