Duration of IV Octreotide for Severe Diarrhea

Treatment Duration

  • The American Society of Clinical Oncology recommends continuing IV octreotide until the patient has been diarrhea-free for 24 hours, typically requiring 2-7 days of treatment 1, 2
  • For patients with severe diarrhea, the standard treatment duration is 7 days, with monitoring of response daily, as most patients achieve complete resolution within 2-7 days 1

Dosing and Administration

  • The standard approach for dosing is to start at 100-150 μg SC/IV three times daily or 25-50 μg/hour continuous IV infusion, and escalate up to 500 μg three times daily if inadequate response 1, 2, 3, 4
  • A dose escalation strategy is recommended, with titration upward until symptom control is achieved, and a maximum effective dose of 500 μg three times daily 1, 2, 3, 4, 5, 6, 7

Monitoring and Safety

  • Daily assessment is required, with evaluation of stool frequency and volume, monitoring for hyperglycemia, and watching for signs of ileus development, particularly after diarrhea resolves 1, 2
  • Complete blood count and electrolyte monitoring should be maintained throughout treatment 1, 2
