Causes and Mechanisms of Vitreous Hemorrhage

Pathophysiology of Diabetic Retinopathy

  • In patients with proliferative diabetic retinopathy, neovascularization occurs at the inner surface of the retina and into the vitreous due to global retinal ischemia, leading to the formation of fragile new vessels that are prone to bleeding, resulting in vitreous hemorrhage 1, 2
  • The new vessels on or near the optic disc and elsewhere in the retina are fragile and prone to rupture, causing vitreous hemorrhage 1, 2

Posterior Vitreous Detachment and Trauma

  • When the vitreous detaches from the retina and optic nerve head, it can cause vitreous traction at sites of significant vitreoretinal adhesion, potentially causing retinal breaks and subsequent bleeding 3
  • Patients with acute posterior vitreous detachment symptoms have an 8-22% chance of having a retinal tear at initial examination 3
  • Both blunt trauma and penetrating eye injuries can cause vitreous hemorrhage, and trauma-induced posterior vitreous detachment can occur at a younger age than typical age-related posterior vitreous detachment 4

Prevention and Management

  • Early detection and treatment of underlying conditions, particularly diabetic retinopathy, can prevent vitreous hemorrhage 1, 2
  • Regular eye examinations for diabetic patients, even when asymptomatic, can help prevent vitreous hemorrhage 1, 2
  • Prompt evaluation of patients with new symptoms of flashes and floaters, and follow-up examination within 6 weeks for patients with vitreous pigment, vitreous/retinal hemorrhage, or visible vitreoretinal traction, are crucial for preventing complications 3, 4
