Medical Necessity Assessment for Intravenous Immunoglobulin (IVIG) Therapy
Indications and Diagnostic Criteria
- The American Academy of Allergy, Asthma, and Immunology recommends IVIG therapy for primary immunodeficiency diseases with confirmed hypogammaglobulinemia (low antibody levels) and recurrent bacterial infections 1, 2
- The American Society of Hematology recommends IVIG therapy for immune thrombocytopenia (ITP) with platelet count <30,000/mcL and active bleeding or high bleeding risk, or failure of first-line corticosteroid therapy 1, 3
- The European League Against Rheumatism recommends IVIG therapy for systemic lupus erythematosus with severe thrombocytopenia (platelets <30,000/mm³) and inadequate response to high-dose corticosteroids 5
Dosing and Administration
- The standard dosing for IVIG therapy in primary immunodeficiency diseases is 400-600 mg/kg every 3-4 weeks 1, 2
- The standard dosing for IVIG therapy in ITP is 1 g/kg as a single dose or 0.4 g/kg daily for 2-5 days 1, 3
- The standard dosing for IVIG therapy in granulomatosis with polyangiitis is 2 g/kg divided over 5 days 4
Required Documentation for Approval
- Laboratory confirmation of the underlying condition, such as immunoglobulin levels, platelet counts, or antibody titers, is required for IVIG therapy approval 2
- A detailed clinical history, including infection frequency, bleeding episodes, or disease activity, is required for IVIG therapy approval 2
- Previous treatment attempts and responses to first-line therapies must be documented for IVIG therapy approval 2
- Patient weight must be verified to ensure dose appropriateness for IVIG therapy 2
- Exclusion of secondary causes of immunodeficiency or cytopenias is necessary for IVIG therapy approval 1, 2
