Evaluation and Treatment of Hypersomnia in Adults

Initial Clinical Assessment

  • The American Geriatrics Society recommends obtaining detailed information from both the patient and bed partner to capture the full clinical picture, including sleepiness characteristics, narcolepsy symptoms, sleep duration, medication review, substance use, comorbid sleep disorders, and medical/psychiatric conditions 3
  • The American Geriatrics Society suggests administering the Epworth Sleepiness Scale (ESS) to quantify subjective sleepiness, with scores ranging from 0-24, and higher scores indicating greater sleepiness 3

Diagnostic Testing Algorithm

  • The American Geriatrics Society recommends performing a thorough neurological examination and cognitive assessment, as hypersomnia may represent early neurodegenerative disease 3, 1
  • The American Geriatrics Society suggests ordering baseline tests to identify treatable secondary causes, including thyroid stimulating hormone (TSH), liver function tests, complete blood count, and serum chemistry 3, 4, 1, 2

Treatment Approach

  • The American Geriatrics Society recommends optimizing any underlying medical, neurological, or psychiatric disorders before treating hypersomnia directly, and carefully withdrawing sedating medications if possible 3, 4
  • The American Academy of Sleep Medicine suggests maintaining regular sleep-wake schedule with adequate nocturnal sleep time, scheduling two brief 15-20 minute naps, avoiding heavy meals and alcohol use, and avoiding shift work or jobs requiring continuous attention under monotonous conditions 4, 2, 1
  • The American Academy of Sleep Medicine recommends using modafinil as a first-line treatment for excessive daytime sleepiness, with a starting dose of 100 mg once upon awakening, and increasing at weekly intervals as needed 4, 1
  • The American Academy of Sleep Medicine suggests using sodium oxybate for narcolepsy type 1, with a liquid given in 2 divided doses at night, and for idiopathic hypersomnia in the United States 4
