Glycemic Targets for Special Populations
Elderly Patients
- The American Geriatrics Society recommends a target HbA1c of 7.5-8.0% for most elderly patients with diabetes, with 7.0-7.5% reserved for healthy older adults with excellent functional status and few comorbidities 1, 2
- For healthy elderly patients with good functional status and few comorbidities, a target HbA1c of <7.0-7.5% is recommended 1, 3
- The American Geriatrics Society found that intensive control (HbA1c <6.5%) is associated with increased hypoglycemia and mortality in older adults 1, 2
Chronic Kidney Disease Patients
- The KDIGO guideline recommends an individualized HbA1c target between 6.5-8.0% for patients with diabetes and CKD, with most patients targeted at 7.0-8.0% 8
- For CKD stages 1-3, a target HbA1c of 6.5-7.5% may be appropriate if achievable without hypoglycemia risk 8
- For CKD stages 4-5, a target HbA1c of 7.0-8.0% is recommended, recognizing that HbA1c becomes less accurate below eGFR 30 8
- The KDIGO guideline emphasizes that HbA1c accuracy decreases significantly below eGFR 30 mL/min/1.73 m², particularly in dialysis patients receiving erythropoietin-stimulating agents 8
Heart Disease Patients
- The American Heart Association recommends a target HbA1c of 7.0-8.0% for patients with diabetes and heart failure, avoiding both intensive control (<7.0%) and poor control (>8.5%) 5
- For stage A-B heart failure, a target HbA1c of 7.0-7.5% is recommended if safely achievable 5
- For stage C heart failure, a target HbA1c of 7.0-8.0% is recommended 5
- The American Heart Association statement emphasizes that observational data consistently show a U-shaped relationship between HbA1c and mortality in heart failure patients, with the lowest mortality at HbA1c 7.0-8.0% 5
