Management of Recurrent DVTs on Apixaban

Assessment and Treatment

  • The American College of Cardiology recommends ruling out any drug interactions that may reduce apixaban efficacy in patients experiencing recurrent DVT while on apixaban therapy 2
  • The Lancet Oncology suggests assessing for underlying conditions that may increase thrombotic risk, particularly active cancer, in patients with recurrent DVT on apixaban 1
  • Switching to therapeutic dose low-molecular-weight heparin (LMWH) is recommended as the most effective strategy to reduce morbidity and mortality in patients experiencing recurrent DVT while on apixaban, according to The Lancet Oncology 1
  • LMWHs have demonstrated superior efficacy in managing recurrent thrombosis, particularly in patients with cancer-associated thrombosis, as reported in The Lancet Oncology 1
  • The American College of Cardiology recommends considering switching to a different direct oral anticoagulant (DOAC) such as rivaroxaban or edoxaban for patients with recurrent DVT on apixaban 2, 1
  • The American Heart Association, as reported in Circulation, suggests considering warfarin therapy (target INR 2.0-3.0) if DOACs and LMWH are not feasible for patients with recurrent DVT on apixaban 3

Special Considerations

  • For patients with active cancer and recurrent DVT on apixaban, LMWH is strongly preferred, according to The Lancet Oncology 1
  • The National Comprehensive Cancer Network recommends avoiding apixaban and considering dose-adjusted LMWH or warfarin for patients with CrCl <30 mL/min 4
  • Patients with gastrointestinal or genitourinary malignancies may have higher bleeding risk with DOACs and might benefit from LMWH, as suggested by The Lancet Oncology 1

Duration of Treatment

  • The American Heart Association, as reported in Circulation, recommends extended anticoagulation (indefinite duration) for patients with recurrent unprovoked DVT 3
  • Regular reassessment of bleeding risk is necessary for patients on indefinite anticoagulation, according to Circulation 3

Monitoring

  • Regular assessment of bleeding risk factors throughout treatment is recommended, as reported in Circulation 3
