Initial Treatment Protocol for AIDS

Timing of ART Initiation

  • The American Medical Association recommends that ART should be initiated immediately after HIV diagnosis, including at the first clinic visit, if the patient is ready to commit to treatment 1
  • Structural barriers that delay receipt of ART should be removed to allow newly diagnosed persons to receive ART at the first clinic visit after diagnosis, as recommended by the American Medical Association 1
  • For patients with acute HIV infection, immediate ART initiation is strongly recommended by the American Medical Association 2
  • The American Medical Association recommends Bictegravir/tenofovir alafenamide/emtricitabine (BIC/TAF/FTC) as a first-line regimen for most patients, with an evidence rating of AIa 2, 4
  • Dolutegravir plus tenofovir (TAF or TDF)/emtricitabine or lamivudine is also recommended by the American Medical Association as a first-line regimen, with an evidence rating of AIa 1, 2
  • The American Medical Association recommends Dolutegravir/lamivudine (DTG/3TC) as a first-line regimen, but only if HIV RNA level is less than a certain threshold, there is no lamivudine resistance, and no HBV co-infection, with an evidence rating of AIa 4

Special Considerations for ART Initiation

  • For most opportunistic infections, the American Medical Association recommends initiating ART within 2 weeks of starting treatment for the opportunistic infection 1
  • For tuberculosis with CD4 counts greater than or equal to 50/μL, the American Medical Association recommends initiating ART within 2-8 weeks of starting TB treatment 1
  • For cryptococcal meningitis, the American Medical Association recommends initiating ART 2-4 weeks after starting antifungal therapy 1, 2
  • For patients with TB meningitis, the American Medical Association recommends initiating ART when TB meningitis is under control, typically 2-4 weeks after starting TB treatment 2

Baseline Laboratory Testing

  • Before initiating ART, the American Medical Association recommends performing the following tests: CD4+ cell count, HIV RNA level, HIV genotype resistance testing, HLA-B*5701 testing if considering abacavir, hepatitis B and C serology, basic chemistry panel, and liver function tests, although the source of this recommendation is not explicitly stated in the provided text, similar recommendations can be found in 1

Monitoring After ART Initiation

  • The American Medical Association recommends measuring viral load 4-6 weeks after starting ART to assess initial response, although the source of this recommendation is not explicitly stated in the provided text, similar recommendations can be found in 1
  • Once viral suppression is achieved, the American Medical Association recommends monitoring every 3 months until suppression is maintained for at least 1 year, although the source of this recommendation is not explicitly stated in the provided text, similar recommendations can be found in 1
  • After 1 year of viral suppression, monitoring can be reduced to every 6 months, although the source of this recommendation is not explicitly stated in the provided text, similar recommendations can be found in 1

Common Pitfalls to Avoid

  • Delaying ART initiation can lead to poorer outcomes and increased risk of HIV transmission, as stated by the American Medical Association 1
