Etomidate Dosing for Pediatric Conscious Sedation

Dosing Recommendations

  • The American Academy of Pediatrics recommends an initial dose of 0.1-0.2 mg/kg IV administered over 30-60 seconds for pediatric conscious sedation 1
  • The most effective initial dose is 0.2 mg/kg IV, which provides adequate sedation in approximately 60-67% of pediatric patients 1
  • The maximum total dose should not exceed 0.3 mg/kg to minimize adverse effects 2, 3

Administration Protocol

  • Etomidate should be administered IV over 30-60 seconds to minimize side effects 1
  • Pre-treatment with fentanyl or droperidol can reduce myoclonus 2, 4
  • Continuous monitoring of vital signs, oxygen saturation, and level of consciousness is necessary 1, 5

Adverse Effects and Management

  • Respiratory depression occurs in approximately 16% of patients, usually manageable with supplemental oxygen and airway repositioning 1
  • Myoclonus occurs in approximately 4-38% of patients, and can be reduced with pre-treatment with fentanyl 1, 4
  • Oxygen desaturation is reported in 5-39% of patients, with higher risk at doses ≥0.23 mg/kg 3

Monitoring Requirements

  • Continuous pulse oximetry and cardiac monitoring are necessary 1, 5
  • Regular blood pressure measurements and assessment of respiratory rate and effort are also required 1, 5
  • Monitoring of level of consciousness is essential 1, 5

Contraindications and Cautions

  • Higher doses (>0.3 mg/kg) should be avoided due to increased risk of respiratory depression 3
  • Etomidate should be used with caution in patients with adrenal insufficiency 2, 4
  • Resuscitation equipment should be immediately available 1, 5
