Treatment for Dysuria (Painful Urination)
Diagnosis and Treatment
- The American Urological Association recommends empiric antibiotic therapy for dysuria treatment after obtaining a urine culture, with the choice of antibiotic based on local resistance patterns and patient factors 1, 2
- Dysuria is a central symptom of urinary tract infection (UTI) with over 90% accuracy in young women when not accompanied by vaginal symptoms 3
- Urine culture should be obtained before starting antibiotics, especially for recurrent or complicated UTIs 2
Treatment Algorithm for Dysuria
- The European Association of Urology recommends first-line empiric therapy options for uncomplicated UTI in women, including Nitrofurantoin and Trimethoprim-sulfamethoxazole, based on local resistance patterns 1, 2, 4
- The treatment duration for uncomplicated lower UTI in women is 3-5 days, and fluoroquinolones should be avoided for empirical treatment if used in the last 6 months due to resistance risk 1, 2, 4
- For patients with diabetes, structural abnormalities, or immunosuppression, Ciprofloxacin and Trimethoprim-sulfamethoxazole are recommended for 7-14 days, based on local resistance patterns 2, 4
Special Considerations
- Antibiotic therapy should be adjusted based on culture results when available, and symptoms should be reassessed after 48-72 hours of treatment 2, 4
- For persistent symptoms despite treatment, repeat urine culture should be performed before prescribing additional antibiotics 1
- Asymptomatic bacteriuria should not be treated in the general population or in catheterized patients 2, 5
