Valsartan Dosing Guidelines

Introduction to Valsartan Dosing

  • The American College of Cardiology recommends that valsartan be dosed at 80-160 mg once daily initially for hypertension, with titration up to 320 mg once daily as needed, and started at 40 mg twice daily for heart failure, uptitrated to the target dose of 160 mg twice daily 1, 2

Dosing Considerations

  • The target dose for valsartan in heart failure is 160 mg twice daily, which should be achieved through uptitration from 40 mg twice daily to 80 mg twice daily and then to 160 mg twice daily, or to the highest dose tolerated by the patient, with dosing adjustments made no more frequently than every 2 weeks 1, 2
  • To achieve maximal benefits of guideline-directed medical therapy in patients with heart failure with reduced ejection fraction, therapies like valsartan must be initiated and titrated to maximally tolerated doses, with at least 50% of the target dose achieved for adequate treatment effect 1, 2
  • Higher doses of valsartan have provided greater benefits than lower doses in clinical trials, with little evidence that subtarget doses yield survival benefits comparable to target doses 3

Special Patient Considerations

  • Patients with initial contraindications should be reevaluated to determine subsequent eligibility for valsartan therapy 4
  • For patients intolerant of ACE inhibitors, valsartan is a recommended alternative, according to guidelines from the American Heart Association, as published in Circulation 4, 5

Monitoring and Dose Adjustment

  • Monitoring of blood pressure, renal function, and electrolytes is recommended during dose titration of valsartan, with consideration of dosage reduction if symptomatic hypotension or renal dysfunction occurs 2
  • Temporary dose reductions of valsartan may be needed, but efforts should be made to restore target doses when possible to ensure optimal outcomes in heart failure management 3

Valsartan Dosing for Hypertension and Heart Failure

Efficacy of Valsartan

  • The VALIANT trial found valsartan to be noninferior to captopril in post-MI patients 6
  • In patients with chronic heart failure, the American Heart Association recommends considering valsartan as an alternative to ACE inhibitors, with a target dose of 160 mg twice daily, which reduced the combined endpoint of mortality and morbidity by 13.2% compared with placebo in the Val-HeFT trial, although the specific citation for this trial is not provided, the efficacy of valsartan in heart failure is well established 6
