Mechanism of Action and Pharmacology of Ubrogepant

Pharmacological Mechanism

  • Ubrogepant functions as a small-molecule CGRP receptor antagonist that competitively blocks the CGRP receptor after the peptide has already been released during a migraine attack, differing from other migraine therapies, according to MMWR Recommendations and Reports 1, 2
  • Unlike CGRP monoclonal antibodies, ubrogepant directly occupies the receptor-binding site, preventing CGRP from activating the receptor, as reported by MMWR Recommendations and Reports 1, 2
  • Ubrogepant works exclusively through CGRP receptor antagonism without any serotonergic activity, which is clinically significant because its mechanism is believed to be nonvasoconstrictive, potentially carrying lower cardiovascular risks than vasoactive medications like triptans in patients with cardiovascular risk factors, according to MMWR Recommendations and Reports 1, 2

Acute Treatment Indication for Ubrelvy (Ubrogepant)

FDA Approval and Guideline Recommendation

  • The U.S. Food and Drug Administration approves ubrogepant for the acute treatment of migraine with or without aura in adult patients; the U.S. Department of Veterans Affairs and Department of Defense guidelines assign a weak‑for recommendation for its short‑term use in migraine management, indicating limited strength of evidence. 3
