Risk Assessment for Adding Strattera to Wellbutrin and Effexor

Primary Safety Concerns

  • The American Academy of Child and Adolescent Psychiatry recommends careful monitoring for serotonin syndrome when combining venlafaxine, bupropion, and atomoxetine, due to additive serotonergic activity that substantially increases serotonin syndrome risk 1
  • The combination of venlafaxine, bupropion, and atomoxetine can cause serotonin syndrome, which manifests with mental status changes, neuromuscular hyperactivity, and autonomic hyperactivity, and can be fatal if not treated promptly 1

Practical Management Algorithm

  • The American Academy of Child and Adolescent Psychiatry suggests starting atomoxetine at a reduced dose of 40 mg daily or less when bupropion is already on board, to minimize the risk of serotonin syndrome 2
  • The guideline recommends assessing for early serotonin syndrome signs within 24-48 hours of atomoxetine initiation, including agitation, tremor, confusion, myoclonus, and diaphoresis, and monitoring blood pressure and heart rate at baseline, 1 week, and with each dose adjustment 1, 2

Seizure Risk

  • The National Comprehensive Cancer Network recommends avoiding the combination of bupropion, venlafaxine, and atomoxetine in patients with brain metastases, seizure history, or other seizure risk factors, due to the increased risk of seizures 3

Emergency Management

  • The American Academy of Child and Adolescent Psychiatry recommends immediately discontinuing all three medications if serotonin syndrome develops, and providing hospital-based care with continuous cardiac monitoring 1

Safety Assessment of Combining Strattera with Effexor and Wellbutrin

Primary Safety Concerns

  • Venlafaxine has been associated with sustained clinical hypertension and increased pulse, according to the American Academy of Child and Adolescent Psychiatry 4

Cardiovascular and Seizure Risks

  • Bupropion reduces seizure threshold with a 0.1% seizure risk, and this risk may be compounded by the addition of atomoxetine, as reported by the National Comprehensive Cancer Network 5, 6

Alternative Treatment Approaches

  • Bupropion alone has some efficacy for ADHD symptoms and lower sexual dysfunction risk compared to SSRIs/SNRIs, potentially eliminating the need for atomoxetine, as noted by the American College of Physicians 7
