Collagen Supplements and Warfarin Interaction

Introduction to Collagen and Warfarin

  • The American College of Chest Physicians and American Heart Association/American College of Cardiology do not list collagen as a known interacting substance with warfarin 2, 3, 4

Monitoring and Management

  • Check INR within 3-7 days after starting collagen supplements, as anticoagulant effects typically stabilize within 2-7 days of any dietary change 1
  • Monitor INR more frequently for the first 2-4 weeks after initiating collagen supplementation 1
  • Instruct patients to report any signs of bleeding, such as bruising, blood in urine/stool, or excessive bleeding from minor cuts 5
  • The American Geriatrics Society emphasizes that older adults require evaluation of diet and dietary supplements when dosing warfarin 1

Patient Assessment and Risk Factors

  • Assess baseline bleeding risk factors, including age, history of stroke or GI bleeding, renal insufficiency, anemia, or concomitant antiplatelet therapy 5
  • Consider INR stability and indication for anticoagulation when deciding to allow collagen supplementation 1, 2, 6

Treatment Adjustments

  • If INR becomes elevated, omit 1-2 warfarin doses and consider oral vitamin K1 1-2.5 mg if bleeding risk factors are present 4, 5
