Topical Antifungal Treatment for Angular Cheilitis

First-Line Topical Antifungal Options

  • The Infectious Diseases Society of America (IDSA) guidelines recommend clotrimazole applied topically to the affected commissures 2-3 times daily for 7-14 days as first-line therapy for angular cheilitis 1, 2
  • Miconazole cream or gel applied 2-4 times daily for 7-14 days is equally effective for treating angular cheilitis 1, 2
  • Nystatin cream or ointment (100,000 U/g) applied 2-4 times daily can be used as an alternative, though it may be less effective than azoles 1, 2, 3

When to Escalate to Systemic Therapy

  • Oral fluconazole 100-200 mg daily for 7-14 days is recommended for refractory cases of angular cheilitis 1, 2, 3
  • Systemic therapy is particularly indicated when angular cheilitis is part of more extensive oropharyngeal candidiasis 1, 2

Critical Treatment Considerations

  • Treatment duration should be continued for the full 7-14 days even if symptoms improve earlier to prevent recurrence 1, 2, 3
