Thyroid Storm Management
Clinical Presentation
- Thyroid storm is a life-threatening medical emergency characterized by severe acute exacerbation of hyperthyroidism with multiorgan decompensation, according to the American Family Physician 1, 2
Treatment Algorithm
- The American Family Physician recommends that treatment must begin immediately based on clinical suspicion without waiting for laboratory confirmation, as mortality rises significantly with treatment delays 3, 4
- Administer propylthiouracil (PTU) first, as it inhibits both thyroid hormone synthesis AND peripheral conversion of T4 to T3, as suggested by Praxis Medical Insights 3, 4
- Administer saturated potassium iodide solution or sodium iodide 1-2 hours AFTER starting thionamides, according to Praxis Medical Insights 3, 4
- Propranolol 60-80 mg orally every 4-6 hours is the first-line beta-blocker because it also blocks peripheral T4 to T3 conversion, as recommended by Praxis Medical Insights 3, 4
- Esmolol is preferred for hemodynamically unstable patients due to its rapid onset and short half-life allowing careful titration, according to Praxis Medical Insights 3
- Avoid beta-blockers in severe heart failure, as advised by Praxis Medical Insights 3
Supportive Care
- Provide oxygen therapy as needed, according to Praxis Medical Insights 3
- Administer antipyretics (avoid aspirin as it can increase free thyroid hormone), as recommended by Praxis Medical Insights 3
- Aggressive fluid resuscitation with large-bore IV access, according to Praxis Medical Insights 4
Hospitalization and Monitoring
- All patients require hospitalization; severe cases need ICU admission, as stated by Praxis Medical Insights 3, 4
- Obtain immediate endocrinology consultation, according to Praxis Medical Insights 3
- Monitor for cardiac complications including heart failure and arrhythmias, as advised by Praxis Medical Insights 3, 4
- Monitor for agranulocytosis with thionamide use (presents with sore throat and fever), according to American Family Physician and Praxis Medical Insights 5, 4
Special Populations
- Treatment protocol is identical to non-pregnant patients, according to Praxis Medical Insights 3, 4
- Monitor fetal status closely, as recommended by Praxis Medical Insights 3, 4
- Avoid delivery during thyroid storm unless absolutely necessary, according to Praxis Medical Insights 3
- Thyroid storm affects less than 1% of pregnant women with hyperthyroidism but carries high maternal and fetal risk, as stated by American Family Physician 1
Follow-Up
- Monitor thyroid function every 2-3 weeks after initial stabilization, according to Praxis Medical Insights 3, 4
- Watch for transition to hypothyroidism, which commonly occurs after thyroid storm treatment, as advised by Praxis Medical Insights 3, 4
Treatment of Thyroid Storm
Introduction to Thyroid Storm Treatment
- The American Academy of Family Physicians recommends immediate multi-drug therapy for thyroid storm without waiting for laboratory confirmation, as mortality rises significantly with treatment delays and can reach 75% if treatment is postponed 6
- The American Academy of Family Physicians suggests that methimazole 20 mg every 4-6 hours can be used as an alternative to propylthiouracil if it is unavailable, though it lacks the peripheral conversion blocking effect 6
Immediate Treatment Algorithm
- The American Academy of Family Physicians recommends administering saturated solution of potassium iodide (SSKI) 5 drops every 6 hours, or sodium iodide 500-1000 mg IV every 8 hours, as part of the treatment algorithm for thyroid storm 6
- Alternative options for blocking thyroid hormone release include Lugol's solution or lithium if iodine is contraindicated, as recommended by the American Academy of Family Physicians 6
- For hemodynamically unstable patients requiring vasopressor support, the European Society of Cardiology recommends using esmolol with a loading dose of 500 mcg/kg IV over 1 minute, then maintenance infusion starting at 50 mcg/kg/min, titrating up to maximum 300 mcg/kg/min 7, 8
- If beta-blockers are contraindicated, the European Society of Cardiology suggests using diltiazem 15-20 mg (0.25 mg/kg) IV over 2 minutes, then 5-15 mg/h maintenance infusion 7, 8
Essential Supportive Care
- The American Academy of Family Physicians recommends oxygen therapy as needed, antipyretics for fever control, and identifying and treating precipitating factors such as infection, surgery, trauma, labor/delivery, or medication non-adherence 6
- The American Academy of Family Physicians also suggests administering dexamethasone 2 mg IV every 6 hours to reduce peripheral T4 to T3 conversion 6
Special Populations: Pregnancy
- The American Academy of Family Physicians recommends that the treatment protocol for thyroid storm is identical to non-pregnant patients, and that fetal status should be monitored with ultrasound, nonstress testing, or biophysical profile based on gestational age 6
- The American Academy of Family Physicians also advises avoiding delivery during thyroid storm unless absolutely necessary due to high maternal and fetal mortality risk 6
Thyroid Storm Management
Immediate Treatment
- The American Academy of Family Physicians recommends initiating treatment immediately based on clinical diagnosis, as mortality rises significantly with delays, and laboratory confirmation should never postpone therapy 9
- The American Academy of Family Physicians suggests administering propylthiouracil (PTU) as the first-line agent in pregnancy, with a standard loading dose followed by maintenance dosing every 4-6 hours 9
- The European Heart Journal recommends esmolol with a loading dose of 500 mcg/kg IV over 1 minute, then maintenance infusion starting at 50 mcg/kg/min, titrating up to maximum 300 mcg/kg/min for hemodynamically unstable patients 10
- The American Academy of Family Physicians recommends dexamethasone 2 mg IV every 6 hours to reduce peripheral T4 to T3 conversion and address potential adrenal insufficiency 9
Supportive Care
- The American Academy of Family Physicians suggests oxygen therapy as needed 9
- The American Academy of Family Physicians recommends identifying and treating precipitating factors: infection, surgery, trauma, labor/delivery, or medication non-adherence 9
Pregnancy-Specific Management
- The American Academy of Family Physicians recommends monitoring fetal status with ultrasound examination, nonstress testing, or biophysical profile based on gestational age 9
- The American Academy of Family Physicians advises avoiding delivery during thyroid storm unless absolutely necessary, as maternal and fetal mortality risk is extremely high during active crisis 9
Anticoagulation
- The European Heart Journal recommends administering antithrombotic therapy (anticoagulant or aspirin) throughout pregnancy to all patients with atrial fibrillation complicating thyrotoxicosis, except those with lone AF 10
- The European Heart Journal suggests heparin as the preferred anticoagulant in pregnancy, as warfarin crosses the placental barrier and causes teratogenic embryopathy in the first trimester and fetal hemorrhage later 10
