Empagliflozina Dosage in Renal Insufficiency

Introduction to Empagliflozina Dosage

  • The American College of Cardiology recommends a dosage of 10 mg once daily for patients with an estimated glomerular filtration rate (eGFR) ≥45 mL/min/1.73 m², which can be increased to 25 mg if necessary and well tolerated 1

Dosage Recommendations Based on Renal Function

  • The Endocrine Society recommends evaluating renal function before initiating treatment and periodically during treatment, and not initiating empagliflozina in patients with eGFR <45 mL/min/1.73 m² 2

Cardiovascular and Renal Benefits

  • The American College of Cardiology and the American Diabetes Association suggest that the cardiovascular and renal benefits of empagliflozina are maintained even in patients with reduced eGFR, including a significant reduction in cardiovascular mortality (HR 0.62; 95% CI: 0.49-0.77) and hospitalization for heart failure (HR 0.65; 95% CI: 0.50-0.85) 1, 3
  • The American College of Cardiology and the American Diabetes Association also report a significant reduction in the progression of nephropathy (HR 0.61; 95% CI: 0.53-0.70) 3

Special Precautions

  • The Endocrine Society recommends monitoring renal function periodically during treatment and considering temporary suspension of empagliflozina in situations that may lead to fluid loss or hypotension 2
  • The American College of Cardiology notes a higher risk of adverse events related to reduced renal function in elderly patients 1
