Atrial Fibrillation Management for Cataract Surgery

Preoperative Assessment and Anticoagulation

  • The European Society of Cardiology recommends calculating the CHA₂DS₂-VASc score to determine thromboembolic risk, with males having scores ≥2 or females having scores ≥3 requiring anticoagulation regardless of surgical timing 1, 2
  • The American Heart Association suggests assessing current anticoagulation status and ensuring therapeutic dosing, as well as evaluating for hemodynamic stability and adequate rate control 1, 2

Perioperative Anticoagulation Management

  • The American College of Cardiology recommends continuing anticoagulation without interruption for cataract surgery, as it is classified as a low-bleeding-risk procedure where the benefits of continued anticoagulation outweigh bleeding risks 1, 2
  • For patients on DOACs, the European Heart Journal suggests continuing therapy through the perioperative period without interruption 1, 2
  • For patients on warfarin, the American Heart Association recommends maintaining an INR of 2.0-3.0 3, 4

Rate Control Optimization

  • The European Society of Cardiology recommends continuing beta-blockers throughout the perioperative period in all patients without contraindication, as withdrawal increases the risk of perioperative atrial fibrillation 1, 2
  • The American College of Cardiology suggests using alternative agents, such as diltiazem or verapamil, if LVEF >40% and beta-blockers are contraindicated 1, 2

Postoperative Management

  • The American Heart Association recommends maintaining the same anticoagulation strategy postoperatively, continuing anticoagulation indefinitely based on CHA₂DS₂-VASc score, regardless of whether the patient remains in sinus rhythm 1, 2, 5

Risk Factor Optimization

  • The European Society of Cardiology suggests optimizing modifiable bleeding risk factors, including controlling hypertension, avoiding NSAIDs perioperatively, correcting any anemia, and not using bleeding risk scores to withhold anticoagulation 1, 2

Documentation for Fitness

  • The American College of Cardiology recommends documenting the CHA₂DS₂-VASc score, current anticoagulant agent and dosing, confirmation that anticoagulation will continue uninterrupted, adequate rate control status, and absence of hemodynamic instability 1, 2
