Management of Gallbladder Hydrops with Gallstones and Fever

Initial Diagnostic Workup

  • The American College of Surgeons recommends obtaining immediate laboratory studies, including complete blood count, liver function tests, and inflammatory markers, to assess severity of inflammation and sepsis in patients with gallbladder hydrops and fever 2, 3

Immediate Medical Management

  • The Infectious Diseases Society of America recommends initiating broad-spectrum antibiotics immediately upon diagnosis, prior to surgical intervention, in patients with gallbladder hydrops and fever 1
  • The Society of Critical Care Medicine recommends piperacillin/tazobactam as the preferred antibiotic regimen for unstable or critically ill patients with gallbladder hydrops and fever 3, 1

Surgical Intervention

  • The American College of Surgeons recommends early laparoscopic cholecystectomy within 1-3 days of diagnosis as the definitive treatment for gallbladder hydrops with gallstones and fever 1
  • The Society of American Gastrointestinal and Endoscopic Surgeons recommends obtaining intraoperative bile cultures to guide targeted antibiotic therapy in patients with gallbladder hydrops and fever 1

Critical Considerations

  • The American College of Gastroenterology recommends monitoring for signs of cholangitis, including fever, jaundice, and right upper quadrant pain, requiring urgent biliary drainage 4, 5, 6
  • The American Diabetes Association recommends heightened vigilance in diabetic patients with gallbladder hydrops and fever, due to increased risk of infection-related complications and conversion to open cholecystectomy 1
