Ritalin Dosage Titration for Children with ADHD
Initial Dosing and Titration
- The American Academy of Child and Adolescent Psychiatry recommends starting methylphenidate (Ritalin) at a low dose of 5 mg twice daily and increasing by 5-10 mg increments weekly based on clinical response and tolerability, with most children requiring 0.3-0.7 mg/kg per dose given 2-3 times daily 1, 2
- Begin with 5 mg twice daily for immediate-release formulations, with the first dose at 7:30-8:00 AM and the second dose at 11:30 AM-12:00 PM 1
- Increase by 5-10 mg per dose weekly, not total daily dose, based on response 1, 2
Monitoring and Assessment
- Maintain weekly contact by telephone during initial titration to assess both efficacy and side effects 4
- Obtain ADHD rating scales from both teachers and parents before each dose adjustment 3, 4
- Systematically ask about specific side effects: insomnia, decreased appetite, headaches, irritability, emotional lability, and social withdrawal 2, 4
- Monitor weight at each visit as appetite suppression is common 3, 4
Special Considerations
- For preschool children (ages 4-5), use lower starting doses with smaller incremental increases due to slower metabolism rates 3
- Behavior therapy should be first-line treatment for at least 9 months before medication in preschool children 3
- Screen for substance abuse symptoms before initiating treatment in adolescents 3
- Monitor for medication diversion to peers or family members in adolescents 3
