Apixaban Dosing and Management

Introduction to Apixaban Therapy

  • The European Society of Cardiology recommends a standard dose of 5 mg twice daily for atrial fibrillation, with potential dose reduction to 2.5 mg twice daily if certain criteria are met 3
  • For acute VTE treatment, the initial dose is 10 mg twice daily for 7 days, followed by 5 mg twice daily, as suggested by the European Heart Journal 3
  • The American Heart Association implies that extended VTE prevention may involve a dose of 2.5 mg twice daily after completing at least 6 months of treatment, based on guidelines from the European Heart Journal 4, 3

Dose Adjustment and Monitoring

  • In patients with renal impairment (creatinine clearance 15-29 mL/min), the dose should be verified to ensure the patient is on the appropriate 2.5 mg twice daily dose, according to the European Respiratory Review 2
  • The European Respiratory Review suggests that drug-drug interactions, such as those with combined P-glycoprotein and strong CYP3A4 inhibitors, may require dose reduction, with a 50% reduction recommended for strong dual inhibitors 1
  • For patients on apixaban, concomitant antiplatelet therapy should be used with caution due to the substantially increased bleeding risk, as noted in Circulation: Arrhythmia and Electrophysiology 7

Special Considerations

  • The European Heart Journal recommends reassessing renal function at least annually or when clinically indicated, especially in patients on apixaban 2
  • In cases of active major bleeding, apixaban should be discontinued, and reversal with andexanet alfa may be considered if life-threatening, although this is not directly cited, the management strategy is implied by the context of apixaban therapy 1
  • For cancer patients, the European Respiratory Review suggests that small molecule inhibitors like alectinib and ceritinib may require apixaban dose reductions, with 25% and 50% reductions recommended, respectively 1
