Management of Insomnia in Patients on Aripiprazole

  • The American Academy of Sleep Medicine recommends doxepin 3-6 mg as a first-line option for sleep maintenance insomnia, due to its superior efficacy and minimal drug interactions, with improvements in sleep efficiency, sleep latency, total sleep time, and sleep quality, reducing wake after sleep onset by 22-23 minutes compared to placebo 1, 3
  • For sleep onset insomnia, ramelteon 8 mg is recommended, with zero addiction potential and no drug interactions with aripiprazole, specifically effective for sleep onset insomnia 3, 2
  • Eszopiclone 2-3 mg is also recommended, addressing both sleep onset and maintenance, with moderate-to-large improvements in sleep quality compared to placebo, and a mean improvement of 28-57 minutes in total sleep time 1, 3
  • Zolpidem 10 mg (5 mg if elderly) is effective for both sleep onset and maintenance, with a mean improvement of 29 minutes in total sleep time and 25-minute reduction in wake after sleep onset 1, 3

Medications to Avoid

  • The American Academy of Sleep Medicine explicitly recommends against trazodone for insomnia, due to its modest improvements in sleep parameters and lack of improvement in subjective sleep quality, with harms outweighing benefits 3, 2
  • Diphenhydramine and other antihistamines should be avoided due to their lack of efficacy data, strong anticholinergic effects, and risks of confusion, urinary retention, fall risk, and daytime sedation 1, 3, 2
  • Benzodiazepines (lorazepam, temazepam) should not be used due to their higher risk of dependency, falls, cognitive impairment, and respiratory depression compared to non-benzodiazepines, particularly when combined with antipsychotics 2

Essential Behavioral Interventions

  • Cognitive Behavioral Therapy for Insomnia (CBT-I) must be initiated alongside any pharmacotherapy, providing superior long-term outcomes with sustained benefits after discontinuation, and should include stimulus control therapy, sleep restriction therapy, and cognitive restructuring 3, 2
  • Sleep hygiene education should supplement treatment, including avoiding caffeine and alcohol in the evening, maintaining consistent sleep-wake times, and limiting daytime naps to 30 minutes before 2 PM 2

Aripiprazole-Specific Considerations

  • Before adding sleep medication, verify that aripiprazole is taken in the morning, the insomnia is not improving after 1-2 weeks of aripiprazole stabilization, and no other medications or substances are contributing to insomnia 1, 2

Implementation Strategy

  • Start doxepin 3 mg at bedtime, which can be increased to 6 mg if insufficient response after 1-2 weeks, and reassess after 1-2 weeks to evaluate efficacy and monitor for adverse effects 2
  • Use the lowest effective dose for the shortest duration, with regular follow-up to assess continued need for medication, and screen for complex sleep behaviors, including sleep-walking, sleep-driving, falls, and cognitive impairment 2
