Ferric Gluconate Dosing Guidelines for Iron Repletion in Hemodialysis Patients

Dosing Schedule (NKF‑K/DOQI)

  • The NKF‑K/DOQI guideline recommends administering 8 weekly doses of 125 mg elemental iron (total 1,000 mg) to hemodialysis patients requiring iron repletion. [1][2]
  • As an alternative lower‑dose regimen, the same guideline permits 8 weekly doses of 62.5 mg (total 500 mg) when a reduced exposure is desired. 1

Dose Interval

  • Each ferric gluconate dose should be given no more frequently than every other day to minimize infusion‑related reactions. 3

Safety Considerations (Kidney International; American Journal of Hematology)

  • Doses >125 mg per administration are linked to a higher incidence and greater severity of adverse events (e.g., hypotension, nausea, infusion reactions) and are not recommended by the manufacturer. [4][5]6

Administration Methods (Kidney International)

  • A 125‑mg dose may be delivered as a slow IV injection at a rate up to 12.5 mg/min (≈10 minutes for the full dose). [4][5]

Extended Repletion Regimens

  • For patients whose iron deficit exceeds 1,000 mg (target ≈ 1,250 mg), the protocol can be extended to 10 weekly doses of 125 mg while still respecting the maximum single‑dose limit. [1][2]
