Respiratory Alkalosis Management

Diagnosis and Causes

  • The American College of Cardiology recommends considering embolia pulmonar, especially if there is sudden disnea, as a potential cause of respiratory alkalosis 4
  • The American Thoracic Society suggests that hyperventilation can be caused by anxiety/panic attack, pain, or syndrome of hyperventilation 4, 5
  • The European Respiratory Society notes that respiratory alkalosis can be caused by pulmonary causes such as asthma/exacerbation asmática, neumonía intersticial, or sepsis 6, 7

Management

  • The American College of Physicians recommends administering potassium supplementation orally or intravenously according to severity, with monitoring of potassium serum levels every 4-6 hours until normalization 1
  • The American Academy of Family Physicians suggests using controlled breathing techniques and tranquilization for hyperventilation psicógena, and avoiding excessive sedation 1
  • The American Thoracic Society recommends using bronchodilators if there is broncoespasmo, and oxygen supplementation only if there is hypoxemia 6, 1

Monitoring

  • The Society of Critical Care Medicine recommends monitoring oximetry of pulse continuously, respiratory frequency every 1-2 hours, and potassium serum levels every 4-6 hours until normalization 7, 6, 1

Errors to Avoid

  • The American College of Physicians warns against administering unnecessary oxygen, as it can worsen alkalosis, and recommends verifying that the sample is arterial for correct interpretation 1, 2
  • The American Thoracic Society advises against assuming a psychogenic cause without ruling out organic causes, such as embolia pulmonar and sepsis 4, 7, 1
