Treatment Approach for Conduct Disorder with Aggressiveness

Psychosocial Interventions

  • Intensive in-home therapies such as multisystemic therapy, wraparound services, and family preservation models should be prioritized as alternatives to residential placement 1
  • Treatment should promote personal responsibility and self-control in patients, teaching them to manage their own aggressive behavior 2
  • Short-term, dramatic interventions like "boot camps" or "shock incarceration" are ineffective and potentially harmful 3

Pharmacological Management

  • Stimulants are first-line treatment when ADHD is present, as they can reduce both ADHD symptoms and antisocial behaviors 4
  • Mood stabilizers like divalproex sodium can be used as adjunctive therapy for aggressive outbursts despite adequate stimulant treatment 4
  • Alpha-agonists can be used as an alternative adjunctive option for aggressive outbursts 4
  • Mood stabilizers like divalproex sodium or lithium have shown efficacy in reducing aggression in conduct disorder 1
  • Atypical antipsychotics, particularly risperidone, may be considered when other options fail 1

Important Clinical Considerations

  • Avoid polypharmacy - try one medication class thoroughly before switching to another 1
  • Monitor medication adherence, compliance, and possible diversion carefully 1
  • Reactive aggression often responds better to mood stabilizers 2
  • Proactive aggression is more challenging to treat and associated with poorer outcomes 3
  • For severe cases requiring chemical restraint in inpatient settings, carefully consider cardiovascular effects and drug interactions 5

Treatment Algorithm

  • Start with comprehensive psychosocial interventions, including family-based therapy 3 and skills training 2
  • Assess for and treat comorbid conditions, such as ADHD with stimulants 4 and mood disorders with mood stabilizers 1
  • For persistent aggression despite above measures, add divalproex sodium (first choice for adjunctive therapy) 4

Cautions and Pitfalls

  • Avoid one-time, short-term interventions that promise quick fixes 3
  • Treatment gains in structured settings may not generalize to community and family settings 3

Pharmacological Management of Aggression and Emotional Dysregulation in Adolescents

Mood Stabilizers

  • The American Academy of Child and Adolescent Psychiatry recommends divalproex sodium as the preferred adjunctive agent for aggressive outbursts in adolescents with conduct disorder and emotional dysregulation, with response rates of 53% for mania and mixed episodes in children/adolescents 6
  • Divalproex sodium can be used adjunctively with stimulants for ADHD patients with persistent aggression, with a maximum dose typically 20-30 mg/kg/day divided BID-TID 6
  • Lithium carbonate is an alternative mood stabilizer with FDA approval for adolescents ≥12 years, though it requires more intensive monitoring and has compliance challenges, particularly if there's a family history of lithium response 7

Atypical Antipsychotics

  • Risperidone has the strongest evidence for reducing aggression when added to stimulants in controlled trials, with a target dose of 0.5-2 mg/day 6
  • Aripiprazole is FDA-approved for irritability in adolescents aged 13-17, with a typical dose of 5-10 mg/day 6
  • Combination of risperidone + lithium or valproate showed efficacy in open-label trials, with critical monitoring for metabolic syndrome risk, movement disorders, and prolactin levels 6

Special Considerations

  • The American Academy of Child and Adolescent Psychiatry suggests that valproate does not significantly interact with venlafaxine, bupropion, or lisdexamfetamine, but carbamazepine would be problematic due to its induction of metabolism of bupropion and venlafaxine 6
  • Lithium + venlafaxine requires monitoring for serotonin syndrome, though the risk is low, and antidepressants can destabilize mood if bipolar disorder is present 7

Treatment Algorithm

  • The treatment algorithm involves optimizing the current regimen, adding a mood stabilizer, and considering atypical antipsychotics if there is an inadequate response, with a minimum trial duration of 6-8 weeks at therapeutic doses/levels before declaring failure 6, 7
