Teicoplanin Dosing in Acute Kidney Injury

Initial Loading Dose

  • The American College of Clinical Pharmacology recommends a loading dose of 25-30 mg/kg for critically ill patients to rapidly achieve therapeutic drug levels due to expanded extracellular volume related to fluid resuscitation 4, 5
  • Loading doses of antimicrobials with low volumes of distribution, including teicoplanin, are not affected by alterations in renal function 4, 5
  • For serious infections, using the higher end of the dosing range, 10-12 mg/kg, for the loading doses is recommended 1, 2

Maintenance Dosing Based on Renal Function

  • For patients with acute kidney injury, teicoplanin should be administered with a maintenance dose of 6-12 mg/kg once daily, with dosing interval adjustments based on renal function severity 1, 2, 3

Important Considerations

  • The Infectious Diseases Society of America notes that pre-dose monitoring of trough concentrations is recommended to ensure therapeutic levels 4
