Critical Safety Concerns with "Tordol" Medication

Introduction to Safety Concerns

  • The American College of Physicians recommends clarifying the medication name "Tordol" before administration, as it may represent a dangerous dosing error, particularly if it refers to Ketorolac (Toradol) 60 mg, which would create a dangerous dual NSAID combination with ibuprofen 1

Contraindications and Precautions

  • The National Comprehensive Cancer Network recommends avoiding the combination of ketorolac with ibuprofen 800 mg, as it is contraindicated and significantly increases the risk of serious adverse events, including additive gastrointestinal toxicity, compounded renal toxicity, and enhanced cardiovascular risk 3, 4, 1
  • The American Heart Association recommends assessing patients for liver disease, renal impairment, GI bleeding risk, seizure history, and respiratory compromise before administering tramadol or ibuprofen, as these conditions may increase the risk of adverse events 5, 3, 4, 1

Dosing Considerations

  • The American College of Clinical Pharmacy recommends dosing ibuprofen 800 mg with caution, as it is the maximum single dose, and typical dosing is 400-600 mg every 6-8 hours, with a maximum daily dose of 2400 mg or 3200 mg depending on formulation 4, 1
  • The American Pain Society recommends dosing tramadol 60 mg within the therapeutic range, with a typical starting dose of 50-100 mg and a maximum daily dose of 400 mg 8, 9, 5

Monitoring Parameters

  • The National Institute for Health and Care Excellence recommends monitoring patients for pain intensity, adverse effects, and breakthrough pain, and reassessing the pain management plan if more than 4 rescue doses are needed in 24 hours 2, 10, 4, 1

Alternative Considerations

  • The American Academy of Pain Medicine recommends considering alternative medications, such as acetaminophen 1000 mg, acetaminophen 650 mg + tramadol 37.5 mg, or tramadol alone, if the combination of ibuprofen and tramadol is inappropriate for the patient 8, 9, 7
