Estradiol Patch Dosing for Feminizing Hormone Therapy

Initial Dosing and Rationale

  • The standard initial dose for feminizing hormone therapy is 50 μg/24-hour estradiol patch applied twice weekly, balancing efficacy with cardiovascular and thromboembolic safety, as recommended by the American College of Physicians 1
  • Trans women undergoing gender-affirming hormone therapy face a two- to fourfold increased risk of cardiovascular mortality and elevated venous thromboembolism (VTE) risk, making lower initial dosing prudent 3
  • The risk of VTE increases with age in trans women, supporting the use of lower initial doses 3

Application Schedule and Dose Titration

  • Apply the 50 μg patch twice weekly (every 3-4 days), changing according to a consistent schedule, to maintain stable serum estradiol levels 4
  • If feminization is inadequate and levels are subtherapeutic, increase to 100 μg/24-hour patches, with a maintenance dose range for transdermal estradiol of 100-200 μg/day 4

Critical Safety Considerations

  • Do not use ethinyl estradiol patches or oral ethinyl estradiol, as this synthetic estrogen carries significantly higher thrombotic risk compared to bioidentical 17β-estradiol, according to the Endocrine Society 3

Progestin Considerations

  • Progestins are not routinely recommended for trans women who have not undergone orchiectomy due to increased cardiovascular risk and breast cancer risk when combined with estrogens, as stated by the World Professional Association for Transgender Health 3

Monitoring Parameters

  • Monitor for signs of VTE (leg swelling, chest pain, shortness of breath) particularly in the first 1-2 years of therapy, and screen for cardiovascular risk factors including blood pressure, lipid profile, and smoking status, as advised by the American Heart Association 2, 3
