Hypertension Management Guidelines
Initial Treatment Approach
- The American Heart Association recommends initiating treatment with a thiazide-type diuretic as monotherapy for most patients with Stage 1 hypertension (140-159/90-99 mmHg) without compelling indications, and for Stage 2 hypertension (≥160/≥100 mmHg), starting with a two-drug combination therapy, typically a thiazide-type diuretic plus an ACE inhibitor, ARB, beta-blocker, or calcium channel blocker 1
- For patients with compelling indications, such as heart failure, post-myocardial infarction, high coronary disease risk, diabetes, chronic kidney disease, and recurrent stroke prevention, select drugs specifically indicated for the compelling condition, and add other antihypertensive classes as needed to reach goal BP 1, 2
Blood Pressure Classification and Treatment Thresholds
- The American College of Cardiology recommends the following blood pressure classification system: Normal BP: <120/80 mmHg, Prehypertension: 120-139/80-89 mmHg, Stage 1 Hypertension: 140-159/90-99 mmHg, and Stage 2 Hypertension: ≥160/≥100 mmHg, with corresponding lifestyle modifications and drug therapy 1
Blood Pressure Goals
- The primary treatment goal is to reduce cardiovascular and renal morbidity and mortality, with a standard goal of <140/90 mmHg for most patients, and a lower goal of <130/80 mmHg for patients with diabetes or chronic kidney disease 1, 5
- Focus on achieving systolic BP goal first, as most patients (especially those ≥50 years) will reach diastolic goal once systolic target is attained 1
Lifestyle Modifications
- All patients with BP ≥120/80 mmHg require health-promoting lifestyle modifications, including weight reduction if overweight, DASH eating plan, sodium restriction to <2.4 g/day, regular physical activity (30-45 minutes most days), and alcohol moderation (≤2 drinks/day for men, ≤1 drink/day for women) 1, 5, 6
Initial Drug Selection Algorithm
- For patients without compelling indications, start with a thiazide-type diuretic as first-line monotherapy for Stage 1 hypertension, and initiate two-drug combination therapy for Stage 2 hypertension, typically a thiazide-type diuretic plus an ACE inhibitor, ARB, beta-blocker, or calcium channel blocker 1
- Alternative acceptable first-line agents include ACE inhibitors, ARBs, beta-blockers, or calcium channel blockers 1
Critical Implementation Points
- When BP is >20/10 mmHg above goal, consider initiating therapy with two agents immediately, with one agent usually being a thiazide-type diuretic 1
- Thiazide-type diuretics enhance efficacy of multidrug regimens, are more affordable than other antihypertensive classes, and have proven to reduce cardiovascular complications in clinical trials 1
