Switching from Cariprazine to Amisulpride

Introduction to Cross-Titration

  • A gradual cross-titration over approximately 4 weeks is recommended when switching from cariprazine to amisulpride, rather than stopping cariprazine and starting amisulpride the next day, to avoid symptom destabilization 1
  • The American College of Psychiatry recommends beginning to reduce cariprazine dose by 25-50% and starting amisulpride at 400 mg/day (or 50 mg twice daily if targeting negative symptoms) during week 1-2 of the cross-titration period 1, 2
  • The cross-titration strategy should continue with reducing cariprazine by another 25-50% and increasing amisulpride toward target dose (400-800 mg/day for positive symptoms; 50-100 mg/day for negative symptoms) during week 2-3 1, 2
  • By week 3-4, cariprazine should be discontinued completely and amisulpride should be at its full therapeutic dose 1, 2

Key Monitoring Points and Target Dosing

  • During the switch, monitoring for symptom breakthrough is crucial, particularly in weeks 2-4 when both medications are at subtherapeutic levels, as recommended by the National Institute of Mental Health 1
  • For predominantly positive symptoms, a starting dose of 400-800 mg/day of amisulpride is recommended, with evidence supporting 800 mg/day from day one for maximal efficacy, according to the British Association for Psychopharmacology 2
  • For predominantly negative symptoms, using 50 mg twice daily (100 mg/day total) of amisulpride is recommended, as it selectively blocks presynaptic D2/D3 autoreceptors, enhancing dopamine release, as suggested by the European College of Neuropsychopharmacology 3, 4
