Management of Microcytic Anemia with Low Hemoglobin

Initial Diagnostic Workup

  • The European Society of Gastrointestinal Endoscopy recommends serum ferritin as the most specific test for iron deficiency in the absence of inflammation, with a level <15 μg/L indicating absent iron stores, while <30 μg/L generally indicates low body iron stores 1
  • In the presence of inflammation, serum ferritin up to 100 μg/L may still be consistent with iron deficiency, according to the European Crohn's and Colitis Organisation 1, 2
  • Additional iron studies should include serum iron, TIBC, and transferrin saturation, with values <16% suggesting iron deficiency, as recommended by the American Gastroenterological Association 2

Differential Diagnosis Considerations

  • The American Society of Hematology suggests considering thalassemia trait if iron studies are normal, with a recommended investigation of hemoglobin electrophoresis to evaluate for thalassemia trait 4

Additional Testing Based on Initial Results

  • If anemia of chronic disease is suspected, the American College of Gastroenterology recommends evaluating for underlying inflammatory conditions, with low iron, low TIBC, and normal/high ferritin 1, 3

Treatment Approach

  • The National Institute for Health and Care Excellence recommends considering adding ascorbic acid to improve iron absorption in resistant cases of iron deficiency anemia 4

Important Clinical Considerations

  • The European Society of Gastrointestinal Endoscopy states that the combination of low Hb, low MCH, and low MCHC strongly suggests iron deficiency as the most likely diagnosis, and recommends investigating the underlying cause of iron deficiency, particularly focusing on gastrointestinal sources of blood loss in adults 1
  • The American Society of Hematology suggests considering specialized testing only if initial iron studies are inconclusive or response to therapy is inadequate, with a recommended investigation of underlying causes of iron deficiency 4, 3
